What Is Breast Reconstruction?
Breast reconstruction is a surgery (or series of surgeries) that rebuilds the breast after breast cancer treatment, most commonly following mastectomy or lumpectomy. The goal is to restore a natural shape, symmetry, and softness to the breast, so you can feel like yourself again.
Reconstruction may include rebuilding the nipple and areola, contouring the chest wall, and adjusting the opposite breast for balance. The process can be done at the time of mastectomy (immediate reconstruction) or delayed until after cancer treatment is complete.

DIEP Flap Reconstruction
This is the most common form of breast reconstruction for our practice, as Dr. Murillo specializes in microvascular surgery. This approach uses the tissue from your lower abdomen to rebuild a soft, natural-feeling breast.
Benefits of DIEP Flap Reconstruction:
- What is typically thrown away in a tummy tuck, is used to create a new breast.
- This flap will grow into the mastectomy skin to provide a support structure that gives the breast a ‘perky’, more youthful appearance. This breast will support itself rather than ‘hang’ in the thin mastectomy skin like an implant.
- Permanent results that do not require implant replacement
- No risk of implant rupturing, rippling, or replacement
- Tummy tuck effect due to using the lower abdominal skin and fat to recreate the breast
Considerations for DIEP Flap Reconstruction:
- 2-3 day inpatient hospital stay is required
- Longer surgical time than implant-based reconstruction
- Due to the tummy tuck, you will be required to walk slightly bent at the waist for two weeks, as with any cosmetic tummy tuck, to allow for incisional healing
- Recovery time is expected to take 4 weeks.
Implant-Based Reconstruction
This is the most common form of breast reconstruction and may be ideal for women who do not have a significant amount of abdominal tissue for DIEP flap reconstruction. Implant-based reconstruction typically involves a two-step surgical process:
- Step 1: A tissue expander is placed beneath the breast skin and chest muscle at the time of mastectomy or later. The implant will rest for four weeks to allow the incision to heal. After the first four weeks of rest, the implant will be expanded weekly until full. The implant will then rest for four more weeks to allow the skin time to adjust to the new size of the breast.
- Step 2: Once the area is ready, the expander is removed and replaced with a silicone or saline breast implant.

- Shorter surgery and faster recovery compared to flap-based techniques
- A good choice for women who don’t have enough donor tissue or prefer not to use it
- Can be broken into multiple stages to accommodate a busy lifestyle or radiation needs
Considerations for implant-based breast reconstruction:
- You might note rippling in the implant based on the density of the remaining mastectomy skin
- Implants should ideally be replaced every 10 years
- Possible capsular contracture over time or with radiation.
- Active nicotine use can result in poor wound healing. However, the risk of wound breakdown over the implant is higher with ongoing nicotine use than with a flap.
Fat Grafting and Revisions
Fat grafting is a minimally invasive enhancement technique that can be used as a standalone procedure or to refine other types of reconstruction. It involves using gentle liposuction to remove fat from donor areas such as the thighs, flanks, or abdomen. The fat is then purified and re-injected into the breast to smooth contours, fill hollow areas, or correct asymmetry.

In some cases, more than one session may be recommended to achieve optimal fullness and contour—especially in areas affected by radiation. The results are soft and subtle, and because it uses your own fat, there’s no risk of rejection. Most women are excellent candidates as long as there’s enough donor fat available.
Nipple and Areola Reconstruction
The final step for many women in the reconstruction process is restoring the nipple and areola. For patients whose nipples were removed during mastectomy, we offer two compassionate, artistic options:
- Surgical Nipple Creation: Small flaps of skin from the breast are used to create a raised nipple structure.
- 3D Nipple and Areola Tattooing: Medical tattooing gives beautiful, natural-looking color and dimension, with shading that mimics a real nipple even if no projection is present.

Both surgical and tattoo options are optional, low-risk, and can be tailored to your preferences and timing.
